Document Checklist (submit copies only)
- Federal income tax returns (1040 forms) for two previous years. If you did not file or need duplicates, contact the IRS at (800) 829-1040 or http://www.irs.gov/individuals/get-transcript
NOTE: NYS returns are not required - W2s from all employers for the previous two years
- Year-to-date profit and loss statement (Self-employed only)
- Pay stubs for the last 30 days for all working household members
- Most current 3 months bank statements for all accounts. Must have your name, bank name, and account number on the statements.
- SSI / SSD award letter from the previous year for you or anyone in the household (if applicable)
- Complete copy of bankruptcy paperwork, including discharge (if applicable)
- Divorce / separation agreement (if applicable)
- Court order or copies of checks for child support (if applicable)
- Copy of driver’s license or non-driver ID
- Copy of marriage certificate (if applicable)
- Credit report fee (check or money order in the amount of $12.82 for 1 person, 25.76 for 2 persons)
- Application fee: $150.00
- Copy of current mortgage statement and current deed (rehab loans only)
- Current property tax bills (rehab loans only)
- 3 estimates for rehab work
- Copy of homeowner’s insurance policy (rehab only)
